Preparing For Adulthood

Preparation for Adulthood (PFA) is a term used to support the transition from childhood to adulthood for children and young people with special educational needs and disabilities (SEND), and to support them to achieve the best outcomes. The four areas of PFA are:

  • Employment & Careers
  • Independent Living
  • Friendships, Relationships & Community 
  • Good Health

Planning for preparing for the transition to adulthood should start from Year 9 but ideally sooner.  For young people with Education, Health and Care Plans, transition will be person centred and focus on the strengths and aspirations of the child or young person.  

At Stormont House we want our Students and Parents/Carers to begin preparing for adulthood as soon as they join us in Year 7. Students are supported to do this through:

  • Weekly PfA lessons that target the four PfA areas and develop students life skills.
  • Setting of PfA outcomes/targets through the Annual Review Process.
  • In House MDT (Multi Disciplinary Team) support and intervention e.g. Speech and Language Therapy, CAMHs, Art and Drama Therapy etc.
  • Access to Community services and activities e.g. Hackney Health & Wellbeing Education, Prevention, and Outreach 
  • Trips out in the community to cafes, libraries, museums, service providers etc.
  • Whole School Careers Programme that supports students to explore careers and the world of work, make informed Post 16 choices and access our Yr 12 Transition Programme, working with Partner FE (Further Education) and AP (Alternative Provision) providers.
